Rahel Berhane, MD: Pediatrician, Medical Director of Children’s Comprehensive Care; pediatric gastroenterologist, sick visits, well checks, rotates after hours call.	Maria Rodriguez, LMSW: Social Worker, bilingual, basic needs assistance/referrals, emotional/crisis support, system navigation and problem solving (Medicaid, SSI, MTP), referrals to community or specialized services, patient/family advocacy, school advocacy. Sonja Bradley, RN: Nurse Case Manager, support with equipment and supplies, home health providers, and therapy referrals, assists with sick visits, prescription refills, well checks, immunizations, and clinical support by phone. Ext 80931	Kendall Sharp, RN CPNP PC/AC: Clinical Operations Manager, Pediatric Nurse Practitioner; supervises clinical staff, manages day-to-day operations, oversees implementation of projects directed at coordinating care and improving the quality/effectiveness of our services.
Anne Claire Hickman, CCLS, CIMI: Child Life Specialist; developmental/behavioral assessments, coordination with schools, sibling and patient support sessions, pain management, parent education, and infant massage education. Ext 80932	Mark Shen, MD: Pediatrician, pediatric hospitalist, Medical Director of DCMC Hospitalist Program; sick visits, well checks, rotates after hours call, and provides continuity of care in the hospital when our patients are hospitalized.
Valerie MacLaurin, RN CPNP PC/AC: Pediatric Nurse Practitioner, lead clinical provider; sick visits, well checks, clinical support by phone, and rotates after hours call.
